IMMUNIZATION REQUIREMENTS

FOR STUDENTS ENTERING THE BATH SCHOOL SYSTEM

            All parents registering a student in the Bath Public Schools must show written proof that their child has received the following immunizations per State of Maine Immunization Law, before they may attend school in Bath. 

·        5 DPT/DT’s (4 if the fourth dose was given after age 4)

·        4 POLIO (3 if the third dose was given after age 4)

·        2 MMR

·        VARICELLA (vaccination or documentation of disease)

Start of school year 2003-04: Kindergarten and grade 1

Start of school year 2004-05: K-2 and grade 9

Start of school year 2005-06: K-3 and 6, 9, 10

Start of school year 2006-07: K-4 and 6, 7, 9, 10, 11

Start of school year 2007-08: K-12

            Acceptable forms of evidence of immunization include previous school health record, medical record from medical office, laboratory results, or official Certificate of Immunization.  One of these must be provided to the school main office for a child to attend Bath schools.
