IMMUNIZATION REQUIREMENTS

FOR STUDENTS ENTERING THE BATH SCHOOL SYSTEM

Beginning in the fall of 1998, all students entering the Bath School System must show written proof that their child has received, at least, 3 DPT's/TD's (one given after the first birth date), 3 Oral Polio (one given after the first birth date), and 2 MMR's (given after the first birth date) before they may attend school in Bath. Recent changes in the State of Maine Immunization Law have tightened up immunization requirements for all children attending Maine Schools.

For a child to attend Bath schools, one of the following must be provided to the school guidance office:

A transcript of the student's immunization record from the previous school.

A copy of your child's immunization record from their doctor.

A copy of an official immunization card.

Dear Parent or Guardian:

Your son/daughter must be in compliance with the State of Maine s Immunization Law before he or she may enter school in Bath. This law states that all students must show written proof of having had:

1. 5 DPT s (Diphtheria, Pertussis and Tetanus or a TD), one given after the first birthdate.

2. 4 Oral Polio, one given after the first birthdate.

3. 2 MMR s (Measles, Mumps and Rubella) given after the first birthday for grades K-9 (one MMR for grades 10-12).

Please provide the guidance secretary with one of the following:

Transcripts of your child's immunization from his/her former school. A copy of an immunization record from your doctor showing proof that all shots have been given.

A copy of an official immunization card.

In order for your child to attend school in Bath, these guidelines must be followed.
